Beyond the Latch: The Role of Physiotherapy, Laser, and Therapeutic Ultrasound in Managing Breastfeeding Mastitis and Cracked Nipples

Breastfeeding is one of the most rewarding experiences of early motherhood, but it can also be one of the most challenging. Nipple damage, breast pain, blocked ducts, and mastitis are common complications that frequently contribute to the premature cessation of breastfeeding.[1] For many mothers, the pain is so severe that they question whether they can continue.
The good news? Physiotherapy offers safe, effective, and non-invasive treatment options that can make a profound difference. Two modalities in particular, Low-Level Laser Therapy (LLLT) and Therapeutic Ultrasound (TUS), are increasingly recognised as powerful tools in the management of breastfeeding-related breast and nipple conditions.
Understanding the Problem
Cracked Nipples
Nipple trauma, including fissures, cracks, erosions, and abrasions, is one of the most common reasons mothers stop breastfeeding early.[2] Pain during latching and feeding can be intense, restricts milk production, causes maternal stress, inhibits oxytocin release, and hinders breastfeeding success.[3] While incorrect latch and positioning are the most common underlying causes, effective healing of the damaged tissue is critical to keeping the breastfeeding journey on track.[4]
Mastitis and Blocked Ducts
Mastitis is inflammation of the breast tissue, now understood as part of a spectrum of conditions ranging from ductal narrowing and engorgement through to inflammatory mastitis, bacterial mastitis, and in severe cases, abscess formation.[5] The Academy of Breastfeeding Medicine's Clinical Protocol #36 (revised 2022) redefined mastitis as a spectrum, recognising that blocked ducts are not truly "blocked" but rather narrowed due to inflammation or changes in the normal breast microbiome (dysbiosis).[5]
If not managed early, these conditions can escalate rapidly, causing significant pain, fever, and in some cases requiring antibiotics or surgical intervention.
Low-Level Laser Therapy (LLLT) for Cracked Nipples
What Is LLLT?
Low-Level Laser Therapy, also known as Photobiomodulation (PBM), uses specific wavelengths of light to stimulate cellular repair, reduce inflammation, and relieve pain.[6] It is non-invasive, painless, leaves no residue on the skin, and is safe for breastfeeding mothers and babies.[7]
What Does the Evidence Say?
The evidence supporting LLLT for breastfeeding-related nipple trauma is growing and encouraging:
A 2025 systematic review and meta-analysis of 7 randomised controlled trials found that PBM resulted in a notable decrease in breastfeeding-related pain, expedited healing of nipple injuries, and improved quality of life compared to controls. Meta-analysis confirmed significantly less sucking pain in the PBM group (OR 0.43, 95% CI 0.23–0.79, p = 0.001).[2][8]
A well-controlled trial cited by NCBI/LactMed found that laser treatment of the nipples resulted in greater nipple pain relief than sham therapy.[6] A meta-analysis of 7 studies confirmed that diode laser light at various frequencies was effective in reducing pain in nipple trauma.[6]
A systematic review of effective interventions for nipple trauma identified low-level laser photobiomodulation as one of the evidence-supported treatments, contributing to tissue repair and pain reduction.[9]
LLLT has been shown to speed healing by 30–50% compared to standard care alone, with many mothers noticing improvement after the very first session.[7]
A 2024 study concluded that PBM was more effective in decreasing nipple pain and inflammation, and subsequently improving milk production and infant weight, than topical treatment alone.[10]
How Does It Work?
Unlike topical creams that coat the surface, LLLT works from the inside out, triggering the body's own cellular repair mechanisms deep within the tissue.[7] It promotes accelerated wound healing, modulates inflammation, and has analgesic effects.[11] It also has antibacterial properties, helping to reduce the risk of infection in cracked skin.[7]
Importantly, LLLT is considered acceptable during breastfeeding by the Drugs and Lactation Database (LactMed).[6]
What Does a Session Look Like?
A typical LLLT session is quick (usually 5–10 minutes) and pain-free. Red light (660 nm) and/or infrared light (808 nm) are applied directly to the nipple trauma site and surrounding tissue.[3] Multiple sessions over consecutive days may be recommended, with evidence suggesting that lower energy levels applied over several sessions produce better pain relief and healing than a single high-energy application.[1]
Therapeutic Ultrasound (TUS) for Mastitis and Blocked Ducts
What Is Therapeutic Ultrasound?
Therapeutic ultrasound uses high-frequency sound waves to generate gentle vibrations in soft tissue, producing thermal energy (deep warmth) and a micro-massage effect.[12] In the context of breastfeeding, TUS is applied directly to the affected area of the breast to reduce inflammation, relieve oedema, and improve milk flow.[13]
Guideline Recognition
In 2022, the Academy of Breastfeeding Medicine (ABM) updated Clinical Protocol #36: The Mastitis Spectrum, and included therapeutic ultrasound as one of the recommended interventions for conditions on the mastitis spectrum.[5] The protocol states:
"Therapeutic ultrasound, or TUS, uses thermal energy to reduce inflammation and relieve edema. TUS may be an effective treatment for conditions arising in the mastitis spectrum. TUS can be performed under the supervision of a trained physician or physiotherapist on a daily basis until relief is achieved."[5]
The recommended breast treatment setting is 1 MHz frequency, continuous mode, intensity of 2.0 W/cm² for 5–8 minutes.[5][14]
What Does the Evidence Say?
A retrospective study by Lavigne et al. (2012) found that women experiencing symptoms of blocked milk ducts who received an average of 3.3 therapeutic ultrasound treatments (8–10 minutes per session) were able to improve milk flow, and the majority reported improvements in breastfeeding and symptoms. No adverse reactions were reported.[15][16]
A 2023 study by Lin et al. explored the effects of combining therapeutic ultrasound with education and massage on breast symptoms in lactating women, demonstrating the value of a multimodal physiotherapy approach.[17]
A 2024 case report from a Portuguese hospital highlighted therapeutic ultrasound as an important therapeutic adjuvant in the management of mastitis.[18]
Important Considerations
It is worth noting that the evidence base for TUS specifically in lactational mastitis, while supportive, remains limited. A commentary by Douglas (2023) noted that early studies showed mixed results, and further well-designed randomised controlled trials are needed to strengthen the evidence.[19] However, TUS continues to be widely used by women's health physiotherapists internationally and is included in the ABM protocol as a recommended intervention.[5]
The Role of Physiotherapy: More Than Just a Modality
What makes physiotherapy uniquely valuable in the management of breastfeeding complications is that we don't just apply a device. We take a comprehensive, holistic approach. A physiotherapy appointment for mastitis or cracked nipples typically includes:
Thorough clinical assessment of the breast, posture, and feeding mechanics[20]
Therapeutic ultrasound to reduce inflammation and improve milk flow[12]
Low-level laser therapy for pain relief and tissue healing[21]
Manual therapy and lymphatic drainage massage to assist with clearing blocked ducts[14]
Kinesio taping to provide breast support and reduce oedema[20]
Education and guidance on latch, positioning, and feeding technique[20]
Referral to lactation consultants and medical professionals where appropriate[20]
Here in Ireland, a number of specialist physiotherapy practices offer both LLLT and TUS for breastfeeding conditions, and the Irish Society of Chartered Physiotherapists (ISCP) recognises breastfeeding support as part of postnatal physiotherapy care.[22][23]
When to Seek Physiotherapy
Mothers should consider seeing a physiotherapist if they experience:
Painful, cracked, or damaged nipples that are not healing
A hard, tender, or red lump in the breast
Breast pain, swelling, or engorgement
Symptoms of a blocked duct (localised pain, reduced milk output)
Early signs of mastitis (redness, warmth, flu-like symptoms)
Early intervention is key. Most cases respond well to one to three sessions, and the sooner treatment is applied, the more effective it tends to be.[24]
If a mother develops a fever, chills, or worsening symptoms despite treatment, she should be advised to see her GP promptly, as antibiotics may be necessary alongside physiotherapy.[13]
Conclusion
Breastfeeding difficulties like cracked nipples, blocked ducts, and mastitis are common, painful, and too often lead to early weaning. But they don't have to. Physiotherapy, with LLLT and therapeutic ultrasound, offers safe, evidence-informed, and effective treatment options that can relieve pain, promote healing, and support mothers to continue their breastfeeding journey with confidence.
